We are excited to announce our plans to introduce multi-disciplinary primary care geriatric clinics in our community. Clinics will be a vital part of our Specialized Geriatric Services (SGS), designed to provide comprehensive and coordinated health services for older adults with complex health conditions, including frailty and dementia.


Overview


Allied Health For Older Adults

Our new geriatric clinic will focus on the health and well-being of older adults in our community. The clinic will offer a range of services to help identify, assess, and treat various health conditions that affect older adults. These services include early screening, thorough assessments, diagnosis, care planning, and treatment. Health professionals from various fields such as Occupational Therapy, Physiotherapy, Social Work, Therapeutic Recreation, will provide their expertise in this clinic. We will also provide support to help older adults transition smoothly through various stages of care with the help of our onsite social worker.


MINT Memory Clinic

This clinic will address concerns with memory and look for possible reversible causes. The Multi-specialty INterprofessional Team (MINT) is a standardized and nationally accredited program specially trained with 10 years of experience. This solution leverages primary care to deliver timely access to high-quality dementia care and support in the community. The MINT team helps older adults stay home longer and out of hospitals and long-term care.

Our goal is to provide integrated health and social care that meets the needs of our aging community. By offering these services, we aim to prevent unnecessary visits to the emergency department, reduce hospitalizations, and avoid premature placement in long-term care facilities. Most importantly, we strive to enhance the quality of life for older adults by helping them maintain their independence and functional abilities.

We believe that this clinic will be a valuable resource for our community, providing the expert care and support that older adults need to live healthier, more fulfilling lives.


Measurable Outcomes:

  1. Patient Health Outcomes: Track improvements in the health and well-being of older adults who use the clinic's services. This can include reductions in emergency department visits, hospitalizations, and premature placements in long-term care facilities.
  2. Patient Satisfaction: Collect feedback from patients and their families to gauge their satisfaction with the care and services provided. This can be done through surveys and interviews.
  3. Functional Abilities: Monitor changes in patients' functional abilities and independence. This can involve assessments of their ability to perform daily activities and maintain their quality of life.
  4. Service Utilization: Analyze the utilization of clinic services, such as the number of patients served, the types of services provided, and the frequency of visits.
  5. Care Coordination: Evaluate the effectiveness of care coordination among the various health professionals involved in the clinic. This can include tracking the timeliness and accuracy of care plans and the level of collaboration between different departments.
